All the tools you need for a high-quality approach in Pulmonary Rehabilitation

Since 2014 our team of clinician/researchers has strived to develop a high-quality offer in Pulmonary Rehabilitation (PR) for patients with chronic pulmonary disease. The goal for this program is to facilitate its use within different settings of practice, including traditional PR centers, as well as satellite sites (i.e., sites that are remote from major institutions) and primary care medical centers, taking advantage of the use of telehealth and web-based resources.

The Canadian Pulmonary Rehabilitation Program has been enriched with the group’s collective experience, previous resources, as well as through consultation and feedback from the larger respiratory community. The program includes web-based comprehensive resources with reference guides for all the elements of PR:

 • pre-program evaluation & reference / physician consultation
 • self-management behavior modification intervention
 • exercise program & prescription
 • post-program evaluation
 • follow-up keeping a healthy lifestyle in the long-term
The scientific team includes leaders in COPD, exercise and rehabilitation in chronic disease, tele-medicine and behaviour modification from renown universities and organizations:

The program is based on the most recent scientific literature. It has also been reviewed and improved by various professional organizations such as the COPD Clinical Assembly of the Canadian Thoracic Society and RESPTREC.
 
The Canadian Pulmonary Rehabilitation Program has incorporated and adapted Living Well with COPD (LWWCOPD) for the self-management education and behavior-modification components, taking advantage of a myriad of existing LWWCOPD education resources. The program includes:
 • standardized learning materials for patients, 
 • reference guides for the healthcare professional describing how the intervention needs to be done (education topics, exercise program), 
 • presentations that can be delivered to patient(s) in-person or via podcast, and
 • a clearly defined plan from the initial evaluation to the maintenance phase.
 
The Canadian Pulmonary Rehabilitation Program supports healthcare professionals with high-quality content and guidance for delivery. Your patients will now obtain consistent messaging across the country.
